Observational ONLY – No Case Responsibilities | • Application (an updated resume) • A Letter of Verification from Institution • An MOU with their home institution • Confidentiality Statement, Participant Waiver, Proof of Rabies & Tetanus Immunization or signed waiver to decline (these will be emailed to you by HR) • Will be enrolled into a zero credit Enrollment (will need additional info for this, HR will reach out for it when needed) NOTE: This only applies if an MOU is obtained. | • $270 fee for background check (If MOU cannot be obtained) • Training fee of $500/week will be waived if visit is two weeks or less OR if there is a chance of reciprocal training • Training fee of $500/week if visit EXCEEDS two weeks OR if a reciprocal agreement is not likely to be obtained • $25 fee for ID Badge (charged no matter the status of MOU) NOTE: All Fees will be invoiced to visitors for payment BEFORE visit |
